Error: 'Cannot insert duplicate key row in object dbo.orc_Tasks with unique index UQ_orc_Tasks'

When opening the plan, after adding some planner tasks to a works order, from SFDC, the following error generates:-

​An unexpected error occurred. Please report this problem to your system administrator with the following information:
System.Data.SqlClient.SqlException: Cannot insert duplicate key row in object 'dbo.orc_Tasks' with unique index 'UQ_orc_Tasks'. The duplicate key value is (11, accbc47d-0ce-4470-a460-7ada66351441, 12804).
The statement has terminated.

The functionality has been enhanced in Orchestrate version 7.77 hotfix 40. Please upgrade to this version or above.
